Reportable Diseases

Surveillance activities at the state and local level are based on specific public health laws, rules and regulations which authorize and require them.  The legal basis for surveillance is often found in State requirements for health-care providers to report to the health department persons with disease of public health importance.

Below, you will find links to important information that references the laws requiring providers to report specific diseases to their local health department, Tennessee's list of reportable diseases, the disease report form that is to be completed by providers, and a direct link to the Tennessee Department of Health's Reportable Disease Website.
