Within Tennessee, our Sullivan County Regional Health Department can issue death certificates for deaths which occurred in that county within the past five years. Certified copies of death certificates are only available at the Blountville Office. In addition, we issue short form birth certificates for births which occurred anywhere in Tennessee since 1949. The certified short-form birth certificates are available from both the Kingsport and Blountville offices for a fee. Short forms for persons born 1950 through 1975 will no longer be acceptable for passports as they do not show parentage. Vital Records can be picked up between 8am-4pm Monday thru Thursday, closed for lunch 12pm - 12:30pm; Friday hours 8am-4pm, closed for lunch 11:30am -12:30pm.

Information needed includes: name, date of birth, mother and father’s names, mother’s maiden name, and persons place of birth
Cost $8.00 for the first certificate and $5.00 each additional certificate of the same person. Checks should be made payable to Treasurer State of Tennessee.

Voluntary Acknowledgement of Paternity
If you are not married when your child is born, your child does not have a legal father. This service allows the father’s name to be placed on the birth certificate for a child born out of wedlock.
