The Vital Records Office of the Sullivan County Regional Health Department offers certified copies of birth certificates and death certificates. Another service provided through the Vital Records Office is Voluntary Acknowledgement of Paternity. At the present time, this service is offered only at our Blountville Office.

Birth certificates issued for anyone born in the State of Tennessee from 1949 to the present year. Identification requirements for Vital Records
Information needed includes: name, date of birth, mother and father’s names, mother’s maiden name, and persons place of birth
Cost $8.00 for the first certificate and $5.00 each additional certificate of the same person. Checks should be made payable to Treasurer State of Tennessee.

Voluntary Acknowledgement of Paternity
If you are not married when your child is born, your child does not have a legal father. This service allows the father’s name to be placed on the birth certificate for a child born out of wedlock.

Death Certificate
Death certificates available to eligible individuals for those who have expired within Sullivan County. Identification requirements for Vital Records
Last five years kept on file in Blountville office only.
Cost is $7.00 for each certificate. Check should be made to Sullivan County Regional Health Department.
Information needed: name, sex, date of death, place of death, funeral home, relationship to the decedent, purpose of copy. Photo ID and other verifying information will be necessary if cause of death is to show on the certificate. Call (423) 279-2780 for more information.
